This is the first step in the care process where we gather information as a preliminary assessment. We then schedule an in home meeting with our client and their family.
During the in-home meeting we further assess the needs of our client and come up with an individualized care plan.

Our individualized care plans are specific to each client. Based on our assessment notes and client needs, we come up with a recommended plan of action.
We then have another meeting with our clients and their families to review the individualized care plan. Once we are all on the same page we move forward with the home caregiver match.

We will set up an in-home assessment or consultation with you and your family at your convenience to determine specific needs and to develop a comprehensive plan of care. We offer a 1-hour per visit minimum which sets us apart. Additional hours and days can be added if the need arises up to full time, 24 hours a day, 7 days a week, 365 days a year.

No. Unlike some companies, 247 Health Care does not lock you into a long-term contract, and you are only charged for the actual hours that one of our caregivers is in your home. You will need to sign a Service Agreement showing our commitment to provide the services you need and the days and times you choose. As needs may change, our nurse will always be available to reassess our clients and provide advice for changes in care. You can change or stop service at any time with no penalties.
